Two of those can stop a caregiver. The rest inform.
Both in the MAR, both because the harm is physical.
A medication that matches a recorded allergy. The stop names the allergen, its severity and its reaction, and it runs even when no allergy is on file, so the absence is stated too.
An as-needed dose before the order’s minimum interval, or past its daily ceiling. The stop shows when the last dose was given and when the next is allowed.
- scheduledAgainst the order in force.
- Six RightsSix boxes, all required.
- allergy checkAlways runs, even where no allergy is on file.Stop 1
- timingBefore the minimum interval, or past the daily ceiling.Stop 2
- witness co-signTeam houses only.
- signedGiven, refused, held or missed, with who and when.
- lockedAddenda only. Never reversed.
Seven states, and exactly two of them stop a caregiver. No module may add a third.
Refused on principle.
Four things Cohort will not do. They are positions, not missing features.
- 01A third stop
Two exist because the harm is physical. A third turns informing into policing.
- 02Deleting anything
The record’s value is that it cannot be quietly changed.
- 03Editing a filed incident
A correction is an addendum under the original.
- 04Recording care in bulk
Each dose, sign-off and completion is signed one at a time.
That is the whole product.
25 capabilities, two stops, one record that cannot be quietly changed.
